IIT Delhi, AIIMS Develop First Hindi Music Based Speech Therapy for Stroke Patients

A collaborative team including researchers from IHBAS, IIT Delhi, and AIIMS New Delhi has published the first Hindi Melodic Intonation Therapy (MIT) in Aphasiology. MIT is a music-based speech rehabilitation program for stroke survivors suffering from aphasia. In a pilot program, MIT practitioners conducted 40 speech therapy sessions and reported speech improvements in all six participating stroke survivors.

 IIT Delhi, AIIMS New Delhi, and the Institute of Human Behaviour and Allied Sciences (IHBAS) have developed India's first Hindi Melodic Intonation Therapy (MIT). This Hindi MIT is a music and speech therapy designed to help stroke patients recover their verbal communication. This breakthrough development in the field of speech rehabilitation for Hindi-speaking individuals has been documented in the pilot study published in the international, peer-reviewed journal Aphasiology.

 This Hindi MIT fills an important gap in post-stroke patient care. While Melodic Intonation Therapy has been utilized successfully in English and numerous Western languages, and in many Asian languages, there existed no validated adaptation of the therapy in Hindi. Unlike other Hindi Mit researchers, who have not merely translated the extant English Mit materials, but have redesigned therapy using the unique rhythm, cadence, and stress patterns of the Hindi language.

 

What is Melodic Intonation Therapy (MIT)?

MIT is a specialized speech therapy that uses melody and rhythm to aid the recovery of language. Aphasia is a communication disorder that results from damage to the brain, and many stroke patients who suffer non-fluent aphasia can sing familiar songs that are frozen in their memory. MIT takes advantage of this ability to recover the patients' communication and speech.

 

Why It Was Necessary to Create a Hindi Version

Millions of people speak Hindi in India, though due to the lack of Hindi speakers around the world, most of the aphasia rehabilitation programs use English as a source language. Hindi speakers engage in rehabilitation programs in English. English and Hindi have distinct differences in sounds, rhythm, and flow in how sentences are formed. Rehabilitation programs in Hindi, which have been created by translating the English programs, are inadequate. To address such challenges, researchers created a Hindi Adapted Framework from Scratch. It shaped the therapies to the natural flow of the Hindi language by developing new melodies and rhythmic exercises.

 

Pilot Study and Its Results

The pilot study was conducted on 6 individuals who had had a stroke and who had Non-Fluent Aphasia and who spoke Hindi. Each participant received 40 therapy sessions that lasted 45 - 60 minutes, and during which each session was monitored by a speech therapist. From the information the researchers provided, the outcome of the program showed that:

  • All individuals who participated in the program showed improvements that were measurable and observed.
  • In addition, students were able to form longer and more fluent sentences, and overall, the participants stated that their quality of life improved.
  • The researchers stated that there were no negative consequences that were observed during the program.

 

Researchers suggested that More Clinical Trials are conducted

Despite the improvement, the researchers who conducted the study stated that with the sample size that was conducted, the study was just a pilot and feasibility trial. Professor Rahul Garg, who is from IIT Delhi, claimed that the improvements stated above were improvements that reflected communication in a broader context. Professor N. M. Anoop Krishnan stated that more clinical trials are expected and that the team is looking for ways to use digital means to broaden the reach so that the therapy can be conducted in locations that are not specialized in this field.

 

Interdisciplinary Collaboration

The project brought together experts from multiple disciplines. The study was led by:

  • IIT Delhi
  • AIIMS New Delhi
  • IHBAS

Researchers from engineering, neurology, ENT, speech therapy and rehabilitation collaborated to develop the language-specific intervention, demonstrating the value of interdisciplinary research in solving healthcare challenges.

 

Why This Innovation Matters

Stroke is among the leading causes of disability in India, and a significant number of survivors experience aphasia, which affects their ability to communicate. Until now, Hindi-speaking patients had limited access to speech rehabilitation tailored to their native language. The newly developed Hindi MIT programme has the potential to improve rehabilitation outcomes while making evidence-based therapy more accessible to a much larger patient population. If validated through larger studies, it could also serve as a model for adapting speech therapies into other Indian languages.

 

Future Scope

The research team plans to expand clinical evaluation through larger multicentre trials and develop digital tools that would allow patients to continue therapy remotely.Such technology could make specialised speech rehabilitation available to patients living far from major hospitals, improving long-term recovery after stroke.

 

Final Thoughts

The development of India's first Hindi version of Melodic Intonation Therapy represents a significant step forward in stroke rehabilitation and speech-language therapy. Although the initial findings are based on a small pilot study, the positive outcomes suggest that language-specific, music-based interventions could substantially improve communication recovery for Hindi-speaking stroke survivors. As larger clinical studies progress, this innovation may reshape neurological rehabilitation across India.
